    Job description

    Cincinnati Health Network is a federally qualified community health center located in downtown Cincinnati providing full service integrated medical care to the Cincinnati area homeless population. We are searching for energetic, caring, service minded Behavioral Health Counselors.

    Due growing demand and our desire to fully assist our patient’s, we wish to hire four new Behavioral Health Counselors – two Counselors will have experience in substance use/addictions and two Counselors will have experience with mental health counseling and coaching. All positions will provide individual and group counseling services to our patients and include assessments, education, treatment planning, advocacy, and follow-up. Our Healthcare for the Homeless Program utilizes a fully inter-disciplinary model of care.

    Behavioral Health Counselor experienced in Substance Use/Addictions

    Responsibilities:

    * Actively participate as a member of the integrated primary care and behavioral health team, working alongside physicians, psychiatrist, nurse practitioners, nurses, behavioral health counselors, and medical assistants.

    * Provide behavioral health screening and assessments to homeless individuals at the McMicken Integrated Care Clinic and other sites.

    * Conduct full screening tests for depression and substance use when pre-screens are positive.

    * Provide counseling and brief intervention activities for behavioral health and substance abuse disorders.

    * Provide brief interventions for chronic disease care such as smoking cessation, diet, exercise, sleep hygiene, stress relief, medication compliance.

    * Participate in Quality Improvement activities.

    * Work alongside physicians and offer behavioral health advice/intervention when needed.

    * Make referrals for primary care and discuss cases with clinicians.

    * Enter patient information and notes in EMR.

    * Make referrals to ongoing sources of care for identified problems.

    * Keep appropriate records and gather data for reporting as required.

    * Other responsibilities as needed.

    Qualifications:

    * Licensed Independent Social Worker or Licensed Professional Clinical Counselor in Ohio.

    * Ability to work effectively with teams and a diverse array of professionals and agencies.

    * Knowledgeable about the region’s MH/SA treatment system and resources.

    * Cultural sensitivity and willingness to work with homeless persons and persons with HIV

    * Commitment to the mission of the Cincinnati Health Network.

    Job Type: Full-time

    Pay: $21.00 - $25.00 per hour
